Registered Nurse

Oversees the patient care plan. Makes visits as needed, but no less frequent than every 2 weeks. Directs care to be provided by the other members of the Interdisciplinary team.

Patients may elect to have their own referring physician follow, or have a hospice physician. Either way, the hospice physician will assure that care provided is palliative and related to the terminal diagnosis.

Social Worker

Assists in many social aspects of care such as nursing facility placement if needed, funeral arrangements, advance directives and support counseling.

Assists the family with spiritual aspects during hospice care, and with bereavement counseling after the patient has died. Chaplains are available to officiate at memorial and funeral services if requested by families.

Volunteers are a very important part of Hospice care. Members of the community as well as family members of former Hospice patients are encouraged to participate in our volunteer program. Volunteers may provide services either in the way of clerical support to the office, or through patient care activities.
